老人腰背痛要防内脏病

　　一、肾脏疾病 许多肾脏疾病都可引起腰痛，常见的有肾盂肾炎、肾结石、肾脏结核、肾下垂和肾盂积水等。肾脏和输尿管的感觉神经纤维，经内脏神经传至第11―12胸神经和第1腰神经后根，故肾和输尿管疾病引起的疼痛，可由肋脊角扩展到下背部及大腿根部的内侧面。肾周围脓肿可引起腰肌痉挛和局部压痛。肾脏肿瘤引起腰背痛，可能与肾的膨胀或脊神经受压有关，故疼痛部位常于右肋脊角。输尿管结石可引起下腰部疼痛、膀胱疼痛。膀胱和前列腺病变则引起骶尾部痛。

　　二、胰腺疾病 胰腺疾病引起的腰背痛可由上腹部放射而来。此外，胰腺包膜薄而不完善，一旦发生病变，特别是胰腺的炎症或肿瘤易波及附近的组织和器官。胰腺的痛觉神经由内脏神经传至第6―11胸神经节。该神经节为双侧性的，急性胰腺炎的急腹症常向左侧腰背部放射，而胰腺癌患者，特别是胰腺体或胰尾部肿瘤。常有顽固难忍的腰背痛，病人常彻夜不能入眠，往往于脊柱屈曲减轻，在坐位时感到舒适，同时伴有食欲减退和体重逐渐减轻等现象，晚期可出现黄疸、消瘦和衰竭。

　　三、盆腔疾病 老年人的尿路感染很常见，但要明确诊断必须采用尿细菌培养计数。女性的盆腔炎可引起腰背部痛，多有下腹部重坠感与压痛，男性前列腺疾病如前列腺肥大或前列腺肿瘤等，疼痛部位主要表现在腰部。
